Piles Grades 1 to 4 — What Each Stage Means and How It’s Treated

Piles Grades 1 to 4 — What Each Stage Means and How It’s Treated

One of the most important things to understand about piles is that the condition exists on a spectrum — and knowing where you fall on that spectrum changes everything about your treatment. The four piles grades run from Grade 1 (barely noticeable, manageable with diet) to Grade 4 (permanently prolapsed, requiring surgery). Treating Grade 3 piles with a cream that only helps Grade 1 is one of the most common reasons patients end up at Chirag Global Hospital after months of ineffective self-treatment.

This guide walks you through all four piles grades in clear, practical language — so you understand your condition and know your options.

What Are Hemorrhoids and Why Are They Graded?

Haemorrhoids are cushions of blood vessels that exist inside and around the anal canal in every human body. They help control bowel movements. When they become swollen, inflamed, or begin to prolapse, the condition becomes what we call piles.

The four piles grades classify internal haemorrhoids based on whether and how far they prolapse — sliding downward and outward from their normal position inside the rectum.

External haemorrhoids (below the dentate line, around the perianal skin) are not graded the same way.

Piles Grade 1 — Inside the Rectum, No Prolapse

Grade 1 haemorrhoids remain entirely within the rectum. The only typical symptom is intermittent bright red blood on toilet paper or in the bowl — and even this may occur only occasionally. Because Grade 1 tissue is above the pain-sensing dentate line, there is usually no pain.

Treatment: Diet changes (high fibre, 2.5–3 litres water daily), elimination of straining, and oral flavonoid medications to strengthen vessel walls. No procedure needed. With consistent lifestyle improvements, Grade 1 piles grades need never progress.

Piles Grade 2 — Prolapse That Returns on Its Own

Grade 2 piles prolapse during straining or a bowel movement but return inside spontaneously once the effort stops. You may feel a momentary protrusion that quickly disappears. Symptoms become more noticeable: intermittent bleeding, itching, mucus discharge, and a feeling of not fully emptying.

Treatment: Lifestyle changes alone are insufficient for the prolapse at this stage. Minimally invasive options — especially laser treatment for piles (Laser Haemorrhoidoplasty) and Infra red coagulation of piles — are highly effective. Surgery is not required for most Grade 2 piles.

Piles Grade 3 — Prolapse Requiring Manual Reduction

Grade 3 is the most common presentation at Chirag Global Hospital. Haemorrhoids prolapse during bowel movements or physical activity and must be manually pushed back inside or goes inside slowly. Symptoms are significantly more impactful: persistent bleeding that may cause iron-deficiency anaemia, mucus discharge, difficulty sitting, and the daily stress of managing prolapse.

Treatment: A procedure is needed. Laser treatment for piles is effective for selected Grade 3 cases with moderate prolapse. For advanced Grade 3, doppler guided hemorrhoidal artery ligation or harmonic excisions, that are minimally invasive surgical procedures are advised.

Piles Grade 4 — Permanent, Irreducible Prolapse

Grade 4 is the most severe of the piles grades. Haemorrhoids are permanently outside the anus and cannot be manually reduced. The tissue may become thrombosed, causing extreme acute pain. Daily activities — sitting, walking, working — are affected. Significant bleeding, mucus, and hygiene

challenges are common.

Treatment: Surgery is required. (doppler guided hemorrhoidal artery ligation or harmonic excisions)

How Are Piles Grades Determined?

Knowing your grade requires a brief physical examination — you cannot accurately self-diagnose this. The assessment at Chirag Global Hospital takes 5–10 minutes and involves a visual inspection, a 60-second digital rectal examination, along with proctoscopy and an  in-house developed Visual rectoscopy system, which enables the doctors to arrive at a precise diagnosis and the exact degree of piles. No special preparation is required.

Conclusion

Understanding the different piles grades is essential because each stage requires a different approach to treatment. While Grade 1 and early Grade 2 piles can often be managed successfully with lifestyle changes, improved bowel habits, and medication, Grade 3 and Grade 4 piles usually require medical procedures to correct the prolapse and prevent complications.

At Chirag Global Hospital, experienced proctology specialists use modern diagnostic tools and minimally invasive treatments to manage piles effectively at every stage. Whether your condition requires dietary guidance, laser treatment, or surgery, the right treatment plan begins with accurate grading.

If you are experiencing symptoms such as rectal bleeding, itching, prolapse, or discomfort during bowel movements, a quick consultation can help determine your piles grade and the best next step for treatment. Early care can prevent the condition from progressing and help you return to a comfortable, symptom-free life.

Frequently Asked Questions

Can piles grades worsen over time without treatment?

Yes. Grade 1 can progress to Grade 2 over months to years if lifestyle habits are not improved. Grade 2 treated only with creams can reach Grade 3. Early treatment and lifestyle modification significantly reduce the risk of grade progression.

Can Grade 3 piles reduce back to Grade 2 on their own?

No. Lifestyle improvements reduce symptoms and prevent further progression, but will not mechanically reduce the grade. Actual grade improvement requires a procedural or surgical intervention.

Is Grade 3 always painful?

Not always. Grade 3 haemorrhoids can cause significant discomfort through prolapse and mucus discharge without causing acute pain — unless thrombosis occurs or a concurrent fissure is present.

Do I need a colonoscopy to confirm my piles grade?

No. Proctoscopy along with Visual rectoscopy — a brief, simple in-clinic examination — is sufficient to grade internal haemorrhoids. A colonoscopy is a separate investigation recommended for patients with specific additional risk factors.
